– 1988

The 1980s constituted the decade of the consolidation of ideas and projects developed by important universities and doctors regarding medical robotics in the surgical area. The complementary relationship between surgical innovators, biomedical engineers, optical improvements, and robotics technology continues to define the frontier of minimally invasive surgery. Kurt Semm, a German gynecologist, was influential in advancing laparoscopy. One of his early achievements was the creation of an automatic insufflator for measuring CO2 pneumoperitoneum pressure. (1)

Semm conducted the first laparoscopic appendectomy, demonstrating that laparoscopy had progressed from a diagnostic to a curative tool. However, laparoscopy was surrounded by controversy during this time due to higher complication rates.(2)

On April 11, at the Memorial Medical Center in Long Beach, California, the first recorded robotic surgical procedure — a CT-guided brain biopsy – took place. A Unimation PUMA 200 industrial robot was utilized to place a probe for a brain biopsy under CT guidance. The idea was to use a solid mechanical framework to hold a guide in place so that a probe could be implanted to reach a surgical target deep within the brain in a straight line while avoiding crucial brain structures. As a result, the procedure is completed faster than with a manually adjusted frame. (3) Furthermore, in 1985, Muhe performed the first laparoscopic cholecystectomy. These first scopes were challenging to handle as one surgeon had to hold the scope with one hand to peer through it.(4)

The first video-guided laparoscopic cholecystectomy was performed by Philippe Mouret in Lyon, France. For the first time in laparoscopy, operating surgeons and everyone in the operating room could watch the video images from the scope on the screen. Videotaping capability is followed as a method of documentation and educational use. This was followed almost immediately by reports of successful laparoscopic cholecystectomies from Dubois in Paris and then news of laparoscopic cholecystectomies by Muhe.(4)

Barry McKernan and William Saye were able to perform the first laparoscopic cholecystectomy in the United States. This led to an explosive acceptance of laparoscopic procedures.(5)
